What Is Primary Home Care (PHC)?
Primary Home Care is a nontechnical, medically related personal care service that is available to eligible individuals whose health problems cause them to have limitations in performing activities of daily living, according to a practitioner's statement of medical need.
Caring For You Is Our Priority
Your home is your sanctuary. You have tailored it to be just as you want it. As you age, keeping up with your home and daily routines can start becoming a little more difficult. At EverHope we aim to make sure you don’t have to struggle to complete those everyday tasks. Our caregivers can help with everyday tasks such as light housekeeping, medication reminders, doctor’s appointment reminders, meal preparation, etc. Our priority is your well-being and promote a healthy living at your own home.
Cost & Coverage
We currently accept private pay and Medicaid (Title XIX) for eligible clients.
Medicaid eligibility and service authorization are verified prior to the start of services.
Private Pay: All in-home care services are billed on an hourly base. The numbers you require can be changed at any time. We require a minimum of 8 hours per week.
